Senior Transition Checklist

A structured, step-by-step checklist to help families organize every aspect of a senior living transition.

Phase 1: Early Planning and Conversations

Begin with honest family conversations about preferences, concerns, and timing. Identify who will take the lead on coordination and establish a shared communication channel. Document the senior's current living situation, care needs, and personal priorities.

Research available options in the preferred geographic area. Understand the differences between independent living, assisted living, memory care, and continuing care communities so the family can evaluate options from an informed position.

Phase 2: Financial and Legal Readiness

Review current financial resources including savings, income, insurance, and potential benefits such as VA Aid and Attendance. Create a projected monthly budget for the new living arrangement and identify any gaps.

Confirm that legal documents are current: power of attorney, healthcare directives, and estate planning documents. If anything is missing or outdated, consult with a qualified legal professional well before the move.

Phase 3: Housing Evaluation and Selection

Visit prospective communities or housing options. Prepare a list of questions covering care levels, costs, activities, staff ratios, and policies. Take notes and compare options systematically rather than relying on first impressions alone.

Involve the senior in visits whenever possible. Their comfort and sense of connection with a place matters enormously for a successful transition.

Phase 4: Move Preparation and Execution

Develop a downsizing plan: sort belongings, digitize important items, and arrange donations or distributions to family. Hire professional movers experienced with senior transitions if the scope warrants it.

Prepare an essentials bag for the first few days: medications, important documents, comfort items, and a change of clothes. Coordinate with the new community on move-in logistics, setup requirements, and orientation schedules.
